These Services may collect personal data, including HIPAA-protected health information (PHI), but do not constitute medical advice. Use of our platform does not establish a doctor-patient relationship.

2. Services & Eligibility

Services Include:

• Lead opt-in forms and landing pages
• Data collection and transmission to licensed providers
• Coordination with telehealth and in-person healthcare providers
• Appointment scheduling and communication support
• Secure transfer of health and personal data to appropriate parties

Eligibility Requirements:

• You must be at least 18 years old or have legal guardian consent
• You must reside in a jurisdiction where our services are available

3. Disclaimers & Limitation of Liability

• No HIPAA Enforcement Liability: We are not your healthcare provider. Data concerns should be addressed directly with your provider.
• Limitation of Liability: We are not responsible for indirect, incidental, or consequential damages. Our total liability will not exceed the amount you paid for services in the previous 12 months.
• Indemnity: You agree to indemnify and hold us harmless for any misuse, violation of laws, or breach of these Terms.
• Security Measures: We use encryption and access controls, but cannot guarantee absolute security.

4. Data Recording & Sharing

• Calls may be recorded for training, quality assurance, or clinical coordination.
• Recordings and data may be shared with providers under strict confidentiality agreements.
• We may also share data with affiliates and partners under binding contracts to maintain security and confidentiality.

5. Ownership & Acceptable Use

All content, software, and intellectual property remain the property of Master Snapshot. You are granted a limited, revocable license for personal use. Unauthorized or abusive use may result in termination and legal action.

6. Dispute Resolution & Governing Law

• Governing Law: These Terms are governed by the laws of the State of California.
• Arbitration Agreement: All disputes shall be resolved through binding arbitration in San Francisco, CA under AAA rules. You waive rights to jury trials and class actions.
